  1. 2 Year History of Address – Rented or Owned.
  1. 2 Year History of Employment – Include Names of Companies & Addresses.
  1. Gross Monthly Income for both Husband & Wife (before taxes):

     ·        Monthly Salaries
·        Commissions
·        Car Allowance & Expenses
·        Bonuses
·        Pensions
·        Social Security or Disability Income
·        Interest & Dividend Income
·        Other Income
·        Self-Employment Income (if self-employed, bring 2 years of Income Tax  returns & a current operating statement and balance sheet) 

  1. Social Security Numbers (Husband & Wife).
  1. Home & Business Telephone Numbers.
  1. A Current Listing of Assets:

     ·        Cash on Hand & in Banks with All Bank Account Numbers
·        Stocks & Bonds
·        Vested Interest in Retirement & IRA’s
·        Cash Value of Life Insurance Policies
·        Automobiles & Vehicles
·        Approximate Value of Furniture & Personal Property
·        Other Assets (Boats, Motorcycles, Campers, Coins Collection, etc.)
·        Real Estate Owned 

  1. A Current Listing of all debts with account numbers (include charge cards) & Show monthly Payments and Balances Due.
  1. Previous Credit References – If Needed.
  1. Divorce Decree & Child Support Ledger – If Applicable.

 Most Lenders require that $350 be paid at the time of the interview to pay the cost of the Credit Report & Appraisal.
